Included in this Folder are all the required forms for Enrollment and Registration.  I thank you for choosing Brighter Beginnings in Child Care for your child's education and socio-emotional development and look forward to working with your child and your family. Each file is either required by the State of California Community Care Licensing Division, Options for Learning, Child 360, DCFS, the Child Care Consortium of Los Angeles, Los Angeles County Office of Education, the Federal Food Program, or the Brighter Beginnings in Child Care Program...Although some forms seem redundant it is imperative that we have all documents in place to maintain a high quality program and to ensure that each child receives emergency medical care if ever there is an emergency. Your children are my first priority...In addition to the enrollment forms, I also need a Birth certificate, copies of your child's medical card,  and  a current immunization card upon enrollment for all children.
